Instructions

Because this entity is treated as transparent for tax purposes, GlobeTax needs to “look through” to the underlying beneficial owners in order to request Certificates of Residency and file reclaims in certain markets.

We require the following for each underlying beneficial owner:

• Legal name

• Tax Identification Number

• Physical mailing address

• Entity type

• Percentage of ownership

This information is required on a per year basis. Please provide a listing for each year from inception, or at most 5 years of historical data. Upon receiving the listing, we will begin creating the required documentation for the underlying beneficial owners.  
Please note that all fields can be populated by manual entry or by cutting and pasting existing information. Please note that special characters (e.g., _, [, ], \, %) will cause an error and are not permitted.

Field Descriptions

*Indicates mandatory field ‡Indicates mandatory field for U.S. Residents only

• Full Legal Name of the Beneficial Owner* = The legal name as it appears on the beneficial owner’s tax return.

• Beneficial Owner's Address Line 1* = The permanent physical address of the entity. Use Address (line 2) only if necessary. Please do not enter P.O. Box Addresses.

• Country of Address* = Select the full name of the country found in the drop down list that corresponds to the Address provided.

• Beneficial Owner's Country of Tax Residency* = Select the full name of the country found in the drop down list that reflects the Beneficial owner's country of tax residency.

• Beneficial Owner's Entity Type* = Select the entity type in the dropdown list that best describes the Beneficial Owner (e.g. Individual, Corporation, Pension, etc.).

• Tax Identification Number‡ = The tax Identification number for the beneficial owner.

• Pension Plan Number ‡ = A three-digit number assigned by the employer or plan sponsor that is used to identify a particular plan.

• Percentage of Ownership* = The beneficial owner’s ownership interest in the fund/trust, expressed as a percentage for a specified year. Please ensure that the total ownership across all beneficial owners equals 100% and does not deviate more than 0.000001%.

• Sublist Worksheet* = Enter the name of the worksheet that contains the applicable sublist for the corresponding row.
