Instructions
Please fill out the Beneficiary Listing template attached. This information is necessary to file reclaims for transparent entities (e.g., partnerships) in specific markets. The attached file must be completed with the names of investors, tax ID, physical mailing addresses, entity types, and percentages of ownership as requested by foreign tax authorities. This information is required annually and should cover from your investment's inception up to 5 years of historical data unless otherwise instructed. Please provide a separate listing for each year. Once we receive the completed listing, we will prepare the necessary documentation for the underlying investors.

Field Descriptions
Full Legal Name of the Beneficial Owner* = The legal name as shown on their tax return
Beneficial Owner's Address Line 1* = The permanent physical address of your entity. Use Address (line 2) only if necessary. Please do not enter P.O. Box Addresses
Country of Address* = Select the full name of the country found in the drop down list for the Address provided.
Beneficial Owner's Country of Tax Residency* = Select the full name of the country found in the drop down list for the Beneficial owner's country of tax residency.
Beneficial Owner's Entity Type* = Select the full name found in the drop down list of the type of entity that best describes the Beneficial Owner (e.g. Individual, Corporation, Pension, etc.)
Tax Identification Number‡ = Tax Identification number for the beneficial owner. Pension Plan Number ‡ = The Plan Number is a three-digit number assigned to a plan by the employer or plan sponsor and is used to identify a particular plan.
Percentage of Ownership* = The stake each beneficial owner has in a fund/trust expressed by a percentage for a specified year. Please ensure the total sum of the percentage of ownership is 100% and does not deviate more than 0.000001%.
Sublist Worksheet* = Enter the name of the worksheet that contains the sublist for the appropriate row.
